Forms & Policies

We aim to provide a fun atmosphere in which students learn the basics of their sport and develop as members of a team. These goals are easily met when all participants – students, parents, and coaches – follow the guidelines of our NCA Athletic Policy.  
 

Student-Athlete and Parent Requirements

Online sports registration is required for each student-athlete who wishes to play an NCA sport. At the time of registration, each family is required to acknowledge and accept the Athlete's Code of Conduct and Parent's Code of Conduct.  
 
Each student-athlete is also required to submit an annual physical form to the school office prior to participating in athletics. You are welcome to submit the physical form provided by your doctor’s office or use the physical form provided below.  
 
If your son or daughter participates in girls’ volleyball or basketball ALL PARENTS ARE REQUIRED TO WORK IN THE NCA GYMS DURING THESE SEASONS. This allows us to offer athletic programs at reasonable prices. Attending your child's games and working in the gym enables you to meet other NCA parents and students. You become more involved in the community and show your children that you value them and their school. You must work on the days your child's team is assigned.
 
Coach Requirements:
Coaches are required to acknowledge and accept the Coach's Code of Conduct. Coaches are also required to complete the Concussion Protocol and submit a certificate of completion to the NCA Athletic Director.
 
Additionally, all coaches are required by the Archdiocese of Chicago Handbook to complete the following steps. Please refer to the Compliance Guidelines and Training Chart below to complete all required items for coaches.
  1. Register and attend Virtus Training,and submit certificate of completion. Please follow the directions below to complete your registration.
     
    Within the registration you will be required to complete and submit a CANTS form (this form needs to be updated yearly), review and respond to the Code of Conduct for Church Personnel and Archdiocese of Chicago Declarations. Lastly, you will be asked to complete a background check.
  2. Register and complete Mandated Reporter Training,and submit certificate of completion. Recertification every five years.
